Our aides are eager to help improve your quality of life by offering their professional services. These include but are not limited to: Assistance with personal hygiene, dressing, and bathing. They may also perform tasks around the house to help patients with their daily needs.

Our medical social workers use their vast knowledge to assist in bridging the gap between our clients and all of the resources and services they need. Their main goal is to support both the client and the client's family. 

Often times a medical crisis or accident can have lasting effects on a person's body. Our speech pathologists are an integral part of our team, providing services to assist our clients in overcoming challenges that include swallowing, speech, language, and cognition.  

 The goal of our entire team is to provide the best and most supportive services, while helping clients to regain control of their lives. Often time when tragedy strikes, overall mobility can suffer. The physical therapists on our team are prepared to assist you or your love one in overcoming these obstacles and regaining their control and confidence. 

Occupational Therapy can be viewed as one of the pillars of in home care overall. Our occupational therapists can be pivotal in a patients overall success in recovery and regaining independence. Some areas that we evaluate and treat are neurological, musculoskeletal, orthopedic, fine motor skills, and more. 

 Our home health aides are trained to assist patients with activities of daily living such as eating, bathing, grooming and taking vital signs. Under the supervision of our nurses and therapists, home health aides also support certain medical care activities like taking a patient’s pulse or blood pressure. 

Speech therapy focuses on helping patients maximize their skills and abilities to live independently. Our speech language pathologists are experts in oral and motor cognition and help patients improve their swallowing, mental processing and language skills. 

 Our social workers assess the social and emotional factors affecting each patient and their loved ones, and then assist in coordinating available community resources and short-term counseling. 

Falls pose a significant risk to older adults. Our services include fall prevention which educates individuals on how to prevent falls and injuries, thus improving their chances to remain at home.

Our goal is to reduce the need for hospital readmissions. Our specialized team is prepared to assist your loved one in coping with their condition, but also to encourage them to live a satisfying and health life, while living comfortably in their home.

Diabetes is one of the leading diseases in our nation. Diabetes can lead to other major health issues, and for that reason we have professionals to focus on this very need. They are able to not only monitor a patients' blood sugar, but also provide education in living a healthy lifestyle while living with this disease.

As with all of our services, our goal for our patients is live healthy, fulfilled lives in the comfort of their own home. We understand that many people have wound care needs. Our wound care specialists use evidence based treatment to care for patients' needs and also provide education to help not only with their current situation but to prevent future wounds, when able.

We understand that the decision to use Home Health Care may be one made after a significant medical event that caused hospitalizations or other care or treatment. Strong communication and communal relationships between providers are necessary for smooth transitions. During any transition, we take care to provide the utmost professional  care and treatment for our patients, while walking them and their family through this process.

Working in conjunction with your doctors, our team will provide individualized rehabilitation plans, whether it is needed after a surgery or injury.
